Capital City Reentry Solutions LLC provides contracted, community -based  reentry and stabilization services for organizations. Services are short-term, scalable, and designed for rapid deployment.

HEALTHCARE DOCUMENTATION & APPOINTMENT SUPPORT

In addition to reentry stabilization services, Capital City Reentry Solutions provides nurse-led healthcare documentation and appointment preparation support for individuals and community programs.

Many individuals experience delays in housing placement, disability benefits, employment clearance, or treatment services because important health information is incomplete or difficult to communicate during medical appointments. We help clients organize and present accurate information so providers and agencies can make informed decisions.

Services include:
• Medical history and medication record organization
• Provider appointment preparation summaries
• Disability and functional limitation documentation support
• Medical paperwork preparation (FMLA, work, and housing forms)
• Housing and program intake documentation reviews

Appointments are conducted virtually. After the session, clients receive a clear documentation summary they can bring to their physician, employer, case manager, or benefits office.

Capital City Reentry Solutions does not provide medical treatment or replace a licensed healthcare provider. Our role is to assist with communication, organization, and documentation preparation so decisions are not delayed due to incomplete information.
